Job specification: Full-time (1.0 FTE). This is an hourly and non-exempt position, which is grant funded by HRSA. This grant has an end date of 09/30/2026 and contingent upon continued Federal Funding.
Schedule: Monday-Friday, 8am-5pm with occasional evening and weekends
Hourly Rate: $19.00 to $21.00 per hour

Care Ring - a fast-growing and highly respected community organization seeks a passionate individual for the role of Family Health Navigator (FHN) for the A Guided Journey- Healthy Start Enhanced (AGJ-HSE) program. The FHN provides case management, coordination of care and health education to AGJ-HSE program participants. As a trusted member of the community, the FHN will facilitate access to services and work to provide high quality and culturally competent service delivery. The FHN must have a passion for Maternal-Child Health to support improved birth outcomes, and family health well-being. The goal of this role is to increase perinatal health knowledge and self-sufficiency among our participants. We do this through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

Basic Job Functions:
- Identify and enroll eligible participants based on A Guided Journey-Healthy Start Enhanced qualifying criteria.
- Provide basic and moderate case management. Maintains accurate records of program participants
- Collaborate with clients on case plans, reviewing and modifying periodically
- Provides home and community-based care coordination and linkage to community resources to program participants and their families
- Utilize home visitation curriculum and conducts structured, scheduled home and/or outreach visits with pregnant and postpartum participants
- Provide evidence-based and evidence-informed education including, but not limited to: breastfeeding, immunization, safe sleep, family planning, smoking cessation, nutrition, mental health, physical health, stress, domestic violence.
- Conducts risk assessments and screenings with participants and refer to community agencies as necessary.
- Demonstrates open and respectful communication in all interactions with program participants and their families
- Submits participant’s data into database system in a timely manner to meet grant productivity requirements.
- Maintains HIPAA and confidentiality
- Complete all training and ongoing professional development for Community Health Worker (CHW) certification
- Attend meetings, events and conferences
- Follows policies, procedures and best practices of Care Ring and the A Guided Journey model.
- Participates in community outreach events and participants recruitment activities
- Promotes public awareness of Care Ring and A Guided Journey model.
- Reports suspected cases of child and/or adult abuse/neglect/exploitation. Serves as liaison during evaluation and investigation.
- Maintains knowledge of A Guided Journey program
- Documents all patient interactions, assessments, interventions and referrals in a timely manner
- Makes decisions independently and as part of a team using sound analytical and problem solving skills
- Work effectively with culturally diverse participants and colleagues
- Participates in staff meetings and case conferencing
- Utilizes effective communication skills to collaborate with the multidisciplinary teams
- Evaluates participants progress toward goals and documents outcomes
- Performs related duties as assigned or required
- Develops and maintains relationships with community agencies and partners
- Initiative-taker, able to multitask independently, and exercise sound judgment and decision-making
- Proof of Covid-19 vaccination is required as a condition of employment
- High School Diploma or GED
- Must complete and pass the Community Health Worker Certification within 6 months of hire
- Experience in home visiting, maternal child health or working with the underserved population
- Applicants must reside in Mecklenburg County with priority given to candidates living in the following zip codes: 28205, 28206, 28208, 28212, 28216, or 28217
- Consideration/preference will be given to job-related education (i.e., certifications, etc.)
- Valid driver's license and insured automobile
- Excellent written and verbal communication skills
- Comfortable conducting home visits to all areas of Mecklenburg County
- Basic computer skills
- Must be able to work a flexible schedule including some evenings and weekends
- Bilingual-Spanish preferred
- Knowledgeable of services available in the community preferred
